Medical Treatment

1. Contact the Special Medical Team at Cho Ray Hospital to schedule an assessment of  your condition.  The Special Medical Team is located at 201B Nguyen Chi Thanh Street in District 5 of HCMC, and can be contacted by phone at 3856-5703.   Please note that the Consulate ONLY accepts assessments from the Special Medical Team at Cho Ray Hospital, and applicants providing statements from other doctors or organizations will be asked to repeat the process. The assessment must discuss the following:

    • The specifics of your condition;
    • The availability of adequate treatment in Vietnam;
    • Whether treatment methods in the U.S. differ significantly from those in Vietnam.

2. Mail your assessment to a U.S. doctor or certified treatment facility.  This doctor should evaluate your condition in light of this assessment and provide you with the below documents:

  • A letter acknowledging receipt of the assessment from the Special Medical Team;
  • A statement that the doctor or facility has agreed to treat your condition;
  • A breakdown of estimated costs and a timetable for the treatment.

3. Submit evidence that you, your family, or your friends have the financial resources necessary to pay for treatment.  Acceptable evidence could include one or more of the following:

  • A receipt for any pre-treatment deposit paid to the doctor or hospital of choice;
  • Proof of personal health insurance coverage;
  • Complete disclosure of all financial resources for the person or charitable organization funding the treatment.  Disclosure for individuals must include, at a minimum, Complete Income Tax Return Transcripts (formerly form 1722) from the IRS and W-2 statements from employers for the past 3 years, as well as current bank statements. Charitable organizations should provide IRS filings for the previous year, as well as evidence that the organization has sufficient funds to cover the treatment.  The person(s)/organization providing financial assistance must also submit a notarized letter indicating their intention to bear all costs associated with your treatment.

Medical Donor

1. Contact the Special Medical Team at Cho Ray Hospital to schedule the below series of tests.  Please note that submitted results must be less than four weeks old.  The Special Medical Team is located at 201B Nguyen Chi Thanh Street in District 5, Ho Chi Minh City and can be reached by phone at 3856-5703.

 

a)      Blood Typing

d)    Screening for HIV

g)    Screening for Herpes

b) Tissue Cross  Matching

e)    Screening for Hepatitis

h)    Screening for CMV

c)       Renal Arteriogram

f)    Screening for Syphilis
i)    Tissue Typing of HLA

                                      
2. Mail a copy of your results to a physician or medical facility in the United States (note that you must bring the original copy of your medical results to your visa interview).  You should request that the physician or facility provide you with the following documents:

    • A letter stating that they have  reviewed the results of the medical tests performed by the Special Medical Team at Cho Ray Hospital, and determined that the applicant is a suitable donor;
    • A statement indicating that all arrangements for the transplant have been completed, and indicating when, where, and by whom the procedure will be carried out;
    • A written estimate of the costs associated with the procedure;
    • A statement from the recipient’s doctor detailing his/her current medical condition;
      A statement from the physician performing the procedure estimating the length of time necessary for both the procedure and the recovery of the donor (applicant). 

3. Provide evidence that financial arrangements have been made for the proposed procedure, as well as for the applicant’s trip to, and return from, the United States.  This may include, but is not limited to, the following:

    • Proof that the recipient has medical insurance covering all costs (for both recipient and donor) associated with the surgery;
    • Deposit receipts for any pre-payment made to the hospital or physician performing the surgery;
    • Notarized letters from friends, family members, or charitable organization indicating that they will bear all costs associated with the surgery and with the applicant’s travel to/from the United States.  These should be accompanied by Complete Income Tax Return Transcripts (formerly form 1722) from the IRS and W-2 statements from employers for the past 3 years, as well as current bank statements.

In addition to providing the information above, you must demonstrate that your proposed trip will be temporary and you intend to return to your permanent residence in Vietnam/another country upon completion of your surgery.
